The size refers to the fit, see sizing chart at Osprey's website. However, IT DOES AFFECT the volume a little also. The small is the true volume advertised, in this case 65L. and goes up about 2L per size. Med-67L. LG-69L

How do you measure for size

You can visit the Osprey website, there is a guide to measuring for your size. You may need someone to help since you'll be measuring down the center of your back, but it saves you the trouble of driving to an outdoor store to get measured for a product that you don't intend to buy from them.

Are the dimensions of the pack when it is full?

Yes, although I'd say it's a bit deeper than 6 inches when loaded up to the max.

Does it worth the extra price versus the old model?

Yes. Not a big price difference, and you get a rain cover and extra lower compression straps. Over the 8-10 years you’ll use this bag, the extra cost is nil
