It's funny you ask, I was worried about that too. I can hear the stall horn when it goes off, and I can hear slight changes in engine noise. Overall fantastic headset. I would buy it again tomorrow if it deemed necessary.

Anyone know how these fit and work over hearing aids?

It depends on the size of the hearing aids. The headset is made to fit over the ears, but not much more.

Is it compatible with helicopter plugs?
